On July 8, 2025, KOT Airlines Flight 564, a Ro-Embraer E190 flight from Sauthemptona International Airport to Mellor Regional Airport, had a uncontrolled engine failure on their right engine which led to a severe cabin depressurization. However, the Embraer E190 flight was forced a controlled crash landing over a mountain range at one of Greater Rockford's islands, which was successful with only a flight attendant and two passengers died in the crash.
The National Transport Safety Board had investigated the engine failure to be a leaking fuel pipe in the second engine, which caused the said engine to overheat and explode, causing debris from the engine to hit a cabin window and breach the inner cabin, leading to the cabin pressure to decrease drastically and requiring the cabin's oxygen masks to be activated. Mechanical logs revealed that the engineers replaced the fuel pipe on the E190. However, the piping somehow wore thin.
Following the incident, KOT Airlines later compensated at least 100,000 RBX to the family members of the deceased passengers.

Flight 564 was operated by a Ro-Embraer E190, registered as JN-PNF. It is powered by two GE 34-8E-10 turbofan engines with serial number 4938. The plane was nine years old at the time of the accident, with its maiden flight on August 2, 2016. The aircraft itself has had no major incidents prior to this incident, but there was a minor incident in which the right engine of the plane overheated, which mechanics found out that there was a minor leak in the second engine. Replacements were done within two days.[1]

There were 103 occupants onboard, with 98 passengers and 5 crew members (3 flight attendants, 2 flight crew). Almost every occupant on board the flight was a Ro-Poland citizen, while some are from other countries. The flight attendents were from either Ro-Sweden, Ro-Portugal, or Bloxanada, while the pilots were from either Bloxico or Bloxrazil.
- The pilot in command was 34-year-old Captain Arturo Halmash from a village in Bloxico. The captain joined KOT Airlines five months prior the airline commencing its operations in December 2008. He previously flew with AeroBloxico for 3 years and had a total flying time of 44,900 hours, of which 11,200 hours were spent on the Ro-Embraer E-Jet family.
- The co-pilot was 35-year-old First Officer Shanor Ingles from a state in Bloxrazil. The first officer previously become a pilot for Azol before he left the said airline due to multiple incidents, and KOT Airlines is the second airline he worked on as a first officer. He had a total flying time of over 5,000 hours, of which 3,400 hours were spent on the Ro-Embraer E-Jet family.
Flight Chronology Prior To Accident[]
The flight departed from Sauthemptona International at 9:34 CET (8:34 UTC) with no delays and was scheduled to arrive at Mellor Regional at 15:45 CET (14:45 UTC). The E190 took off without an incident. As the plane made its initial ascent, it eventually reached cruising altitude of 32,000 ft at 11:45 CET (10:45 UTC). The flight cruised normally, passing the Island of Sauthemptona and over the oceans of Southern Rockford.
At 13:34 CET (12:34 UTC), the flight was set for descent to Mellor Regional, with the Air Traffic Control at Mellor clearing them for a descent to 12,000 ft.
Accident[]
Engine Explosion during Descent[]
As the pilots was setting the plane for descent, Co-Pilot Ingles noticed an irregular temperature reading on the second engine; it was higher than normal. As Captain Halmash was deciding on what to do, the engine exploded due to it been overheated, causing debris to hit a cabin window, breaking the cabin window and effectively depressurizing the cabin severely.
The pilots immediately ordered the activation of oxygen masks in the cabin while they waited for further instructions.
Crash[]
Mellor International Airport requested Flight 564 to attempt an emergency landing and diversion. The pilots chose to emergency land over the Greater Rockford Hillside near Airbase Garry, which the pilots order for the cabin to brace for Impact They then attempted the landing, but the plane crashed with a touchdown.
The aircraft was skidding over the rocky terrain of the Greater Rockford Hillside; a burning engine was separated from the plane during the crash and was later found eight kilometers away from the crash site. The tail section was separated from the plane and it led to a flight attendant to be killed, before the aircraft came to a stop 9.8 kilometers away from the moment they touched down.
Evacuation of the aircraft was made. The surviving crew were successful in evacuating all survivors, and only two passengers were killed during the crash due to severe injuries.
Witnesses[]
Minutes after the accident, Airbase Garry was notified of the crash and activated emergency response to assist the survivors. Interviewers from GNN reported the incident as "Aircraft Crash Landed at Greater Rockford Hillside". Few witnesses who were hiking along the hillside saw the aircraft coming in from the north and heard a crash before they saw huge black smoke coming out. The hikers went to assist the survivors before medical assistance arrived.[2]

Evidence[]
The FDR and CVR data shows that the plane's right engine was leaking fuel because a replacement pipe broke in the middle of their initial ascent, overheating the engine. The data recovered showed that the overheat alarm was activated four hours after the initial pipe breakage. Passengers recall seeing a bright orange flame coming out of the second engine and them noticing one of the windows was shattered by debris.[3]
It was unknown why the overheat alarm was activated four hours after the pipe broke until intel showed that there was no elements in the engine settings that could trigger a sudden overheat, which could tell that why it did not activate as it took its time to overheat the engine, and during the descent, the engine overheated.
The indication of a broken replacement pipe meant that the maintenance repair was successful but the replacement pipe was unable to fix the damaged piping and caused it to break and leak fuel over the engine turbine.
The NTSB investigated on the E190's maintenance logs and found out the fuel pipe replacement happened two days before the flight due to the previous flight noticing a decrease in pressure of their right engine.

Aftermath[]
After the Incident, KOT Airlines instituted a policy for any mechanical defect on any parts of the plane to be reported to mechanical staff and the flight is to be grounded for four days to a week to perform any necessary repairs.
