On December 7, 2014, Robloxia Airways Flight 955, an E170-200, crashed shortly after takeoff from Pine County Regional Airport, at 12:11:27am ERT, resulting in the deaths of 22 people, along with 4 other ground fatalities.
Aircraft and Crew[]
The aircraft involved was a twin-engine Rombraer E170-200 with the registration "T-174R", a derivative of the Rombraer ERJ line of regional jets. The jet was manufactured in June of 2008 and entered service with Envoy Air and was acquired by Robloxia Airways as a way to make short haul/regional flights from one city from another. The aircraft was featured with two General Electric CF34-8E engines. Near the time of the accident, the aircraft was painted in a special "Spirit of Robloxia" livery, a retro version of the Robloxia Airways livery.
The pilot for flight 955 was 31-year-old Steve H. Smith. He had 394 hours on the E170. The first officer was Randy S. Ansley, aged 29, and he had 42 hours on the E170, although, he did have some minor flaws with Crew Resource Management. There was also a flight attendant on board.
Accident[]
The flight crew began December 6, 2014, 23:30 ERT, in Quill Lake National Airport, operating the incident aircraft as Robloxia Airways Flight 1457, flying from Amaxon International Airport in Amaxon County to Pine County Regional Airport in Pine County.
The aircraft pushed back from gate B7, at 00:03 ERT, and taxied to runway 22C. Since were delayed and fatigued, they accidentally skipped the "Ready for Takeoff" checklist. At 00:09, they got clearance. At 00:11:16, the plane hit rotation speed and took off, but immediately after, the plane drifted towards the right and at 00:11:26, first officer Ansley exclaimed "Jesus!". At 00:11:38, the E170 impacted hangar A3 at an 88-degree angle, with the last words of the pilots being "Throttle, dammit! Stalling! Stalling!".
Investigation[]
Upon arrival, the RTSB immediately knew what was wrong when they combed through the wreckage and found the flaps in the "1" position (these flaps settings are proprietary to the ERJ line of jets). A few days later, the RTSB flew the wreckage back to their base in Robloxia City in Robloxia County. Once they evaluated the CVR and the FDR, they immediately concluded the investigation as "The pilot's failure to do the correct checklist, leading to fatigue". The investigation was now over.
